The COVID-19 vaccine is being offered to all children aged 12 to 15 years of age. Your child has the opportunity to receive their first COVID-19 vaccine, and you may be notified about the second dose later. Please discuss the vaccination with your child and then complete and return the Covid-19 e-consent form to us within 48 hours of receipt, so that we can review your child’s information before the scheduled school session. Your child’s health record will be updated with vaccine information if you consent, and your child receives the vaccination as planned.
